Eyebrow Transplant

Eyebrow transplantation is one the most popular cosmetic treatments today for both men and women. It is also one of the most challenging procedures to perform due to the complicated and delicate nature of eyebrow hairs themselves.

 

An eyebrow hair loss could occur from a multitude of different reasons, through accidents (cuts, burns etc.) or naturally occurring thinning of the eyebrows. In either case, restoring or completely recreating an aesthetically pleasing eyebrow requires years of experience and skill. This is because eyebrow hairs have a very unique character than scalp hairs.

Eyebrow hairs grow at a very acute angle with every individual hair having a unique growth direction and flow which gives eyebrows their shape.

 

Dr. Nagai is one of the pioneers of eyebrow transplant surgery with his presentation of the Nagai eyebrow transplantation method in 2005 in the Italian hair society congress being the first in the world!

In this procedure, Donor hairs are collected from the back of the scalp using The Nagai Method. The amount of donor hairs required varies depending to the extent of the eyebrow hair loss. The Nagai Line-Slit Method for Eyebrow Transplantation

The eyebrow hairs are very unique and therefore several important factors must be considered for a natural looking result.

Eyebrow hairs are single hair follicles with unique individual growth patterns. In fact, no two eyebrow hairs grow in a similar direction to each other. As a result, donor hairs must be carefully chosen to match the eyebrow hair type.

Furthermore, eyebrow hair density is significantly lower than the scalp`s density and therefore any attempt at recreating the eyebrows should take that into account. Interestingly, the eyebrow hair density changes along the eyebrow`s arch with the outer third portion having the lowest density.   

Taking the case of this patient as an example:

 

The patient is a young female who was unsatisfied with her left eyebrow. Considering the extent of reconstruction needed, an initial examination indicated that a 200 graft surgery will achieve a natural looking satisfactory result (as marked).

After extracting the suitable 200 grafts from the back of the scalp, the next step and the most challenging one is creating individual pores and designing a natural looking eyebrow.  

Using micro blades for making new hair pores is common practice among experienced hair transplant surgeons. Despite its advantages, many surgeons still struggle to create a natural looking eyebrow. This is because most surgeons do not consider the unique growth pattern and flow angle of the eyebrow hair follicles in the recipient region. The insertion direction of the micro blade will determine the growth angle and pattern of the transplanted hair.

Dr. Nagai pays close attention to the local hair pattern and tries to replicate a similar design by adjusting the flow and angle of growth for each transplanted hair as seen here. Furthermore, Dr. Nagai uses a much finer micro blade size of 0.7 mm to create a more stable and accurate eyebrow design.

We can see that the cuts made with the micro blades by Nagai are neither horizontal nor vertical. They are made individually and specifically to match the local pattern of a natural looking eyebrow.

Here is a detailed look at the eyebrow transplantation procedure performed with The Nagai Method. Dr. Nagai carefully makes the incisions on each eyebrow following a similar pattern to the local eyebrow hairs, making the result more natural and elegant. Dr. Nagai is able to make accurate incisions using either hand (ambidextrous) which gives him a unique advantage and allows him to make perfect incisions on either side of the patient`s face with similiar percision.

Hair Cycle Discrepancy

Similar to the body hair transplant method, the scalp hair cycle and the eyebrow hair cycle do not match as well. This is evident in the fact that scalp hairs often have a growth phase which lasts for 2 – 7 years whereas eyebrow hair`s growth phase is much shorter at around 6 – 12 months. This means that the transplanted hairs will continue to grow longer than the surrounding eyebrow hairs until their growth cycle comes to an end.

However, this is only temporary. Research shows that the hair follicle receives its growth and rest signals from its surrounding adipose tissues. That means a scalp hair that is transplanted in the eyebrow region will get its growth and rest signals from the local eyebrow tissues and its cycle will eventually match the surrounding eyebrow hairs. As a result, it will behave like a natural eyebrow hair over time.

This adaptation will often take 1 – 2 years to complete based on the previous patient’s experiences. In the meantime, eyebrow transplant patients should groom the transplanted hairs by cutting them to the appropriate length until the transplanted hairs adapt to their new environment.
